DNR / POLST Forms
Do not resuscitate (DNR) and physician orders for life-sustaining treatment (POLST) forms allow individuals to communicate their medical care preferences in advance.
These forms are used to:
- Indicate whether heart and lungs resuscitation should be performed
- Provide instructions for medical care during serious illness or at the end of life
- Guide emergency responders and healthcare providers
Forms must be completed and signed according to state requirements to be valid. It is important to keep these forms accessible and to share them with family members and healthcare providers.
