Medical Records Requests
To request copies of your medical records, please submit an originally signed Authorization to Release Medical Records (Form 2099) (electronic signatures cannot be accepted) and a copy of the patient’s photo ID.
Requests submitted by a third party must also include a check or money order for $15 to cover processing.
Submissions can be emailed to DHS-FAX-PHSMRRequest@SacCounty.gov or faxed to 916-854-9251. Requests are processed in the order received and should be sent within 10 days from the date the request is received.
For additional information or to check on your request after 10 days, you may call 916-874-9298. In order to help us respond to phone calls in a timely manner, please only leave one voicemail message.
