First appointment?

Please arrive at least 30 minutes early to your first appointment to complete necessary forms. 

Please bring:

• ​Medi-Cal, Medicare, or Healthy Partners card
• Valid identification (driver's license, state-issued identification card, passport, etc.)​

After Hours Medical Care

• If you have a question for your doctor and the Sacramento County Health Center is closed, please call (916) 874-9670 and you will ​​be connected to a health professional.
• If you have Anthem Blue Cross, Molina, or HealthNet, you have access to a 24-hour​ nurse advice line.  See information from your health plan.​
