Full Service Partnership (FSP)

Full Service Partnership (FSP) provides intensive, complete mental health services for people in Sacramento County with significant and complex mental health needs. The program supports children and families, young adults, adults, and older adults who need a higher level of support to remain stable in the community. 

Full Service Partnership (FSP) is designed for people who may be at risk of psychiatric hospitalization, incarceration, or homelessness without comprehensive, coordinated care.

What Full Service Partnership (FSP) Does 

Full Service Partnership (FSP) provides coordinated, team-based care that addresses the whole person. This includes mental health, housing, physical health, and daily living needs. 

Full Service Partnership can help by: 

  • Providing individual, family, and group counseling 
  • Offering 24/7 crisis support 
  • Coordinating psychiatric medication services 
  • Delivering intensive case management 
  • Assisting with housing and stability 
  • Supporting employment, education, and benefits access 
  • Connecting people to physical healthcare and community services 
  • Offering peer support from people with lived experience 

Services are focused on the patients’ strengths, culturally responsive, and tailored to each person’s needs. The goal is long-term stability, recovery, and connection to community. 

Full Service Partnership (FSP) is sometimes misunderstood. To help clarify: 

  • Full Service Partnership is not a walk-in clinic 
  • Full Service Partnership does not accept self-referrals 
  • Full Service Partnership is not short-term outpatient therapy 
  • Full Service Partnership is only reserved for people who need a higher level of ongoing support 
  • Full Service Partnership does not replace emergency services in situations involving immediate danger

Full Service Partnership (FSP) serves people who are living with serious mental health challenges and need a high level of coordinated services to remain safe and stable in the community. 

This may include people who: 

  • Have experienced repeated psychiatric hospitalizations 
  • Are involved in or at risk of involvement with the justice system 
  • Are experiencing homelessness or are at risk of homelessness 
  • Have co-occurring substance abuse issues, along with their serious mental health needs 
  • Need more comprehensive support than traditional outpatient services can provide 

Eligibility is determined through a structured screening and referral process. 

Full Service Partnership (FSP) services are delivered by a multidisciplinary team that may include: 

  • Mental health counselors 
  • Case managers 
  • Psychiatrists and medication providers 
  • Peer support specialists 
  • Employment specialists 
  • Housing and community support staff 

The team works together to provide ongoing, coordinated care focused on recovery and stability. 

How Full Service Partnership (FSP) Works

Full Service Partnership (FSP) is accessed through a structured referral process. It is not a walk-in or self-referral program. 

Access to Full Service Partnership may occur through: 

  1. ​​CORE (Community Outreach Recovery Empowerment)

    ​​CORE (Community Outreach Recovery Empowerment) as a community-based entry point

  2. Hospitals and Emergency Departments
  3. Jails and Justice-System Partners
  4. Community Providers
  5. Behavioral Health Services Partners

For adult and young adult services, eligibility is determined through the Intensive Placement Team using the Level of Intensity Screening Tool.

When Full Service Partnership (FSP) May Not Be the Right Option 

Full Service Partnership may not be appropriate when: 

  • Someone is seeking short-term therapy only 
  • The level of support needed does not meet criteria for this level of care 
  • Immediate emergency medical care is required 

In those situations, other ​Behavioral Health Services programs may be more appropriate.
