The Community Health Assessment (CHA) and the Community Health Improvement Plan (CHIP) are two foundational public health planning tools that Sacramento County Public Health uses together to understand and address the health needs of the community. The CHA uses data to understand the health status, needs, and assets of Sacramento County. Think of it as a health "report card" for the county. The CHIP is the strategic action plan that follows the CHA. It translates the data and priorities identified in the assessment into concrete goals, strategies, and interventions for Sacramento Couty residents.
